I have lost my refresh and back buttons on the top of my pages – how do I get them back?

Do you mean on the top of your Safari browser windows? I’ll assume so.
In Safari, choose View, Customize Toolbar.
You can then drag the Refresh and back buttons back into your toolbar, or restore it all by dragging the default set into the toolbar.
