I have a first generation iPad and suddenly a floating icon appeared on the screen. When tapped it opens and I’m able to mute or unmute, control the volume, etc. I don’t know what I did to get it there but my husband has an iPad 2 and wants one on his screen. Any ideas how he might get it?

Perhaps you activated one of the accessibility features of iOS 5 on the iPad? Take a look at episode 621 and see if anything there looks like what you have:
