I notice when I add an address to my Address Book and save the name as say Kathy for the first name, when I want to email that person the name then comes up as Kathy Kathy in Apple Mail.
When I go back to edit to the correct name and save changes it does it again. Sometimes I don’t use the full name, but rather the first name section. Must I use the first and last name blocks? Thanks.

The problem isn't with Address Book. It is in Mail. Mail is using "autofill" to fill in the email address that you have previously used. To clear out that cache of old email addresses, in Mail, go to Windows, Previous Recipients. See if the address is there and choose it, press Remove From List.
That does not work either
So you have removed your names from Previous Recipients and from your Address Book -- it has to be in one of those two places.
each change MUST be saved after it is done before editing the next one and the next one, and perhaps must be done in the "all" group at the top, although I can't say for sure. Hope that helps!
