Add it to iTunes. Then set your iPhone sync settings to sync all ringtones, or just that one.

go to itunes settings ~ activate ringtones as s available sources.
drag the downloaded m3r to itunes.
they will show up under ringtones.
connect iPhone ~ Sync iPhone (if manual sync: select ringtones)
