I cannot figure out how to keep old podcasts from showing up in iTunes. Every time I refresh iTunes, I see old MacMost Now podcasts grayed that have previously been removed by CTRL-DEL. For example, I see episodes 116-140 currently for MacMost, but it is not always the same set up episodes that appear after a refresh.

Sounds like an iTunes bug. If you have deleted them, then they should be gone and not return unless you choose to reload all of the episodes.
I've often run into this issue as well and never found a permanent solution.
