I do that using screen recording software. It doesn't happen live, but is an effect I can apply on the recorded screen recording before exporting. I use Screenflow. If you record with the built-in QuickTime Player app, it can also apply an effect when you click.
However, if you wish to customize the cursor for live presentations, then look at the app "Mouseposé" in the Mac App Store.
