So you don't know how this icon got into your LaunchPad? If that is the case, then the most likely way it happened is you downloaded and installed something you should not have trusted. A search comes up with a few instances where a malicious website has tricked people into downloading what it claims is the "Flash player" from their site. It is not the Flash player. Instead, it was something that installed this icons and perhaps lots of other things on your Mac.
You should be very careful whenever downloading anything online. To install something, you need to give permission (enter your user password). If you give your permission without being sure what it is you are installing, then your machine can be compromised.
Most likely you will find this icon in your system Applications folder and delete it from there. But perhaps not. The installer could have done many things once you gave it permission. I would run the free version of Malwarebytes to check, or have your Mac checked out by a local expert.
