I’m trying to take inventory of assets for my church for a building move. I am in the process of setting up a Apple Numbers form and would like to scan a barcode and take a picture for/of each item into my form using the iPad I’m taking inventory with.

There's no function to do that in Numbers. The app doesn't have a way to scan barcodes, and even if it did, it wouldn't know what the barcodes mean because it doesn't access any database that can translate the numbers on products (I assume you mean books, DVDs, etc) to actual product names.
For that you'll need an app. Go to the App Store on your iPad and search for "barcode inventory" and similar things. See what you can some up with. Look for something that will allow you to scan products and get them into a list, then export that list to a file (text, csv) that can then be imported into a Numbers spreadsheet.
