I assume you are talking about the Apple Maps app on your iOS device. You can't currently alter the route beyond choose from the multiple suggested routes you usually get when starting.
If you need to be able to adjust routes, you could always try another app. The Google Maps app, for instance, won't let you do this with your finger, but it does have an "Add Stop" function. And there are many other maps apps available too, some of which may have what you want.
Or, you could just go with the route suggested, then deviate from it while driving. The route will adjust as you go.
