The only software I know of that will do that is the mother of all PDF software: Adobe Acrobat. This is what most people use, I think. I've never had to do it myself so I'm not sure if there are any cheaper solutions. It is easy to create PDFs in just about anything: Pages, Keynote, Word, TextEdit even. But PDFs with real PDF fields that people can fill in requires pro-level software and Acrobat is definitely the main one here.
Searching for other options, I see a lot of software that mentions editing and adding text, but that is something you can just do in Preview. Only Acrobat I know for sure lets you create real forms.
I use PDF Element to create fillable form fields in a PDF. It gives me the same field options that I have used before in Acrobat, such as word wrap, combs etc. I chose to purchase the perpetual licence version, rather than the rental version.

Kishore: Can you CREATE PDF forms in PDFPen? I haven't used the most recent version, but their website says nothing about creating PDF forms. It says you can fill them out, edit them, and write your own text on them. But that is not what Phyllis is looking for here.
