That's a tough one. Usually you have your photos in iPhoto on a Mac, but there is no iPhoto on a PC. You have iTunes on both, but syncing over the net could be a huge bandwidth hog and problem if you have even a medium-sized collection.
In addition, if you don't have a static IP address at home (and work) or a MobileMe account (for back-to-my-mac) then the two machines will not be able to find each other as their IP address changes. And they could both be behind simple firewalls (your home Cable/DSL router and who-knows-what at work).
This could be a big undertaking if you really want to figure this all out. Not sure how important it is to you. If it was just a small collection of documents, then DropBox might be your solution. But if you have 10,000 photos like I do and a huge music collection, I wouldn't try it.
Use Dropbox!
The problem with DropBox here is that iTunes and iPhoto collections are usually way too big. You'd need to pay a lot extra to get the storage space at DropBox. Plus, even if you paid for it you'd then have issues with bandwidth -- you'd be sending large amounts of files between each computer and the DropBox servers.
